Immigration Medicals
Instructions:
1) Please
telephone to make an
appointment (and bear in mind we are usually booked several weeks
ahead) and then,
2) Please
print and read the Immigration Medical instructions.
PLEASE READ THEM VERY CAREFULLY. IT IS VERY COMMON IN OUR
EXPERIENCE THAT PATIENTS DO NOT FOLLOW OUR INSTRUCTIONS LEADING TO
DELAYS.

(1) Form 26
Read the form.
Complete personal details on Page
1
Attach photograph to Page 1,
staples are fine.
Complete pages 2 & 3 of Form – please be
thorough, include history of all conditions, operations and
procedures, blood transfusions, broken bones etc. Include a full
list of medication, puffers etc including strength and indication.
Do not sign the form.
(2) Form 160
Attach photograph to this form
Go to your nearest X Ray Clinic and have a Chest
X-Ray. The Radiologist is to complete the Form 160 and give it back
to you along with the x-rays. (Any radiology practice is fine,
including Nambour and Noosa Hospital). Anyone 11 years of age or
older must have a chest x-ray. Cost of the x-ray is charged by the
X-ray clinic that you use. Any problems getting X-Rays please ring
us.
(3) Passport
Please bring with you at the time of the medical.
(4) Glasses Prescription
spectacles or contact lenses must be brought with you at the time of
the medical examination. (You will be required to remove your
contact lenses for the eye test. Please bring a container to place
them in)
(5) HIV Test All applicants
intending to stay in Australia permanently, 15 years of age and
over, require a HIV blood test. This will be arranged after the
consultation with the Doctor. Cost of this will be charged by the
pathology office that you use.
(6) Urine Test Upon
arrival you will be required to provide a urine sample test.
(7) Fees Payment of the
medicals is required at the time of consultation. We have EFTPOS/
Credit card facilities or cash. Cheques will not be accepted.
(8) Appointments
Confirming your appointment the day prior is essential. Please
ring the Clinic. In not doing so this appointment will be
cancelled. A $50.00 cancellation fee will be charged if the
appointment is not cancelled.
(9) Interpreter If you need a
translator for the consultations please bring an appropriate person
with you or let the clinic know and we can book a translator through
the Australian Translation Service.
(10) Immunization Any
immunization records you have are to be brought also. For children,
please bring records of childhood immunizations.
(11) Examination You will be asked to
undress down to your underwear for examination by the doctor. A
chaperone will be present.
(12) Additional Costs Any additional costs for tests
required will be met by the patient.
