Over 75's Health Assessment

Once you reach your 75th birthday, the Health Care team at Nambour, Woombye and Palmwoods Clinics will invite you to participate in our annual assessment program. The clinic has a dedicated registered nurse, Carol Lagden for this program. Carol completed her nurse training in 1980 at  the Price of Wales & Prince Henry Hospitals in N.S.W. She has always had an interest in Aged Care nursing and has worked in many of the local residential and nursing homes as an agency nurse. She settled with her family on the Sunshine Coast in 2005.

This assessment can be completed either in your own home or at one of the clinics. As a Commonwealth Government initiative to promote and maintain good health and independence for older people, the assessment will be completed at no cost to you.

The assessment consists of a short interview to determine your level of independence. There will also be a discussion about your general health, family history, recording of recent vaccinations, current medications (including vitamin supplements, eye drops etc.) and basic observations. It should take no more than forty-five minutes to complete.

At the end of the interview we will discuss any findings with you and if required, identify any services that are needed to allow you to continue living independently. We are then able to refer you to the appropriate service providers. We will ask you to make a follow up appointment with your doctor, within two weeks of the assessment, to discuss any recommendations made at the interview.
